SUBSCRIPTION AGREEMENT – WEEKLY
(THIS IS A RECURRING CHARGE. PLEASE READ!)

Cafejo Roasts/FlavorsBy completing the sign-up form below, you are agreeing to purchase a subscription for 12-count cartons of Cafejo brand coffee &/or tea, which will automatically be delivered or shipped to you weekly. Your payment card will be charged weekly until canceled.

To view various pricing options before purchasing, choose a plan from the Subscription Plan pop-up menu below. The amount will immediately update beneath the BUY SUBSCRIPTION NOW button. If the plan you’re looking for is not available, please call us at (714) 822-4747, or email us at chris@goodstuffcoffee.com, and we’ll create a custom plan just for you.

Form Completion Instructions:

  1. CARD HOLDER’S NAME: Enter the Card Holder’s Name exactly as it appears on the payment card. If there’s more than one line, like on a company card, just enter the first line, which is usually the person’s name.
  2. EMAIL ADDRESS: Enter your email address so you will receive a receipt for each transaction.
  3. DELIVERY CONTACT NAME: Enter the name of the person the subscription order will be shipped or delivered to, which may be different then the Card Holder above. If the order will be received at a business location, please enter the Company Name & Person’s Name in this field.
  4. DELIVERY CONTACT PHONE: Enter a phone number where we can reach you or the Delivery Contact if there’s a problem with the delivery of your subscription order. A mobile phone number is preferred, but you may also enter a home or work number. If it is a work number, please enter a direct dial number or include the extension.
  5. DELIVERY ADDRESS: Enter the complete delivery address for the subscription order all on one line, including street number, street name, any apartment or suite number, city, state and zip code.
  6. CAFEJO FLAVOR(S): For Single Serve Kups, enter the Cafejo coffee or tea flavor(s). You may select a different flavor for each 12-count carton if you wish. Please indicate how many 12-count cartons of each flavor. If this field is left blank, your Single Serve Kups will be flavors of our choosing.
  7. SPECIAL INSTRUCTIONS: If there’s something you want us to know or do that isn’t answered in another field, please enter any special instructions or notes here.
  8. SUBSCRIPTION PLAN: Choose your plan from the Subscription Plan pop-up. The amount will immediately update beneath the BUY SUBSCRIPTION NOW button.
    1. All plans include a shipping & handling fee per order based on the time zone of the Delivery Address (PST = Pacific, MST = Mountain, CST = Central, EST = Eastern).
    2. There is no charge for shipping on orders totaling at least $59.
  9. BILLING INFORMATION FIELDS: Enter and double check your billing address and payment card information. The most frequent reason for a failed charge is that the billing address and zip code do not match the payment card.
  10. COUPON CODE: If you have a Coupon Code, enter it and click the APPLY button. A confirmation will display near the top of the form and the amount will be updated beneath the BUY SUBSCRIPTION NOW button.
  11. When you’re sure everything is correct, click the BUY SUBSCRIPTION NOW button.
  12. If the subscription is successfully processed, you will be redirected to a Thank You page and a receipt will be emailed to you. If there is an error, it will be displayed near the top of the form or by the item where the error occurred. You will need to correct the error and click the BUY SUBSCRIPTION NOW button again.
