SUBSCRIPTION AGREEMENT – MONTHLY
(THIS IS A RECURRING CHARGE. PLEASE READ!)
By completing the sign-up form below, you are agreeing to purchase a subscription for 16-oz bags of The Lost Bean brand coffee, which will automatically be delivered or shipped to you monthly. Your payment card will be charged monthly until canceled.
To cancel your subscription, please email us at firstname.lastname@example.org.
To view various pricing options before purchasing, choose a plan from the Subscription Plan pop-up menu below. The amount will immediately update beneath the BUY SUBSCRIPTION NOW button. If the plan you’re looking for is not available, please call us at (714) 822-4747, or email us at email@example.com, and we’ll create a custom plan just for you.

NOTE: After clicking the “Apply” or “Buy Subscription Now” buttons, any success or error messages will display directly below this notice. If you need help completing the Sign-Up Form, please review the instructions at the bottom of this page.
- Enter the Card Holder’s Name exactly as it appears on the payment card.
- Enter your email address so you will receive a receipt for each transaction.
- Enter the name of the person the order will be shipped or delivered to. If to a person at a company location, please enter as Company Name/Contact Name
- Enter a phone number where we can reach you or the Delivery Contact if there’s a problem with the delivery of your subscription order.
- Enter the complete delivery address all on one line, including street number, street name, city, state and zip code.
- If you’d like to receive your coffee already ground, please enter the type of grind (i.e. Drip Grind, French Press, Pour Over, etc.). Otherwise, please enter Whole Bean.
- Enter any special instructions or notes.
- Choose your plan from the Subscription Plan pop-up. The amount will immediately update beneath the BUY SUBSCRIPTION NOW button.
- Enter and double check your billing address and payment card information.
- If you have a Coupon Code, enter it and click the APPLY button. A confirmation will display near the top of the form and the amount will be updated beneath the BUY SUBSCRIPTION NOW button.
- When you’re sure everything is correct, click the BUY SUBSCRIPTION NOW button.
- If the subscription is successfully processed, you will be redirected to a Thank You page and a receipt will be emailed to you. If there is an error, it will be displayed near the top of the form or by the item where the error occurred. You will need to correct the error and click the BUY SUBSCRIPTION NOW button again.
