14 DAY TRIAL – FREE COFFEE SAMPLE

  • Once you sign up for a Trial Subscription Plan using the form below and click the BUY SUBSCRIPTION NOW button, a free 6-oz bag or 6-count sample will be delivered to you within 7 calendar days, and your payment card will NOT be charged for 14 days, allowing you time to try out the sample before your subscription becomes active.
  • If you do NOT cancel this Trial Subscription within 14 days, your payment card WILL be charged and your subscription will be activated.
  • All subscriptions are open-ended, so you can still cancel at any time without penalty, but no refunds will be given once your payment card has been charged.
  • Please indicate which FREE SAMPLE you would like to receive in the form below (either “Ground” for a 6-oz bag, or “Single Serve Kups” for a 6-count sample).

Form Completion Instructions:

  1. CARD HOLDER’S NAME: Enter the Card Holder’s Name exactly as it appears on the payment card. If there’s more than one line, like on a company card, just enter the first line, which is usually the person’s name.
  2. EMAIL ADDRESS: Enter your email address so you will receive a receipt for each transaction.
  3. DELIVERY CONTACT NAME: Enter the name of the person the subscription order will be shipped or delivered to, which may be different then the Card Holder above. If the order will be received at a business location, please enter the Company Name & Person’s Name in this field.
  4. DELIVERY CONTACT PHONE: Enter a phone number where we can reach you or the Delivery Contact if there’s a problem with the delivery of your subscription order. A mobile phone number is preferred, but you may also enter a home or work number. If it is a work number, please enter a direct dial number or include the extension.
  5. DELIVERY ADDRESS: Enter the complete delivery address for the subscription order all on one line, including street number, street name, any apartment or suite number, city, state and zip code.
  6. GRIND TYPE OR SINGLE SERVE FLAVOR(S): Note that this field is for your subscription order, NOT for your free samples.
    1. For Single Serve Kups, enter the Cafejo coffee or tea flavor(s). You may select a different flavor for each 12-count carton if you wish. Please indicate how many 12-count cartons of each flavor.
    2. For Bagged coffee, if you’d like to receive your coffee already ground, please enter the type of grind (i.e. Drip Grind, French Press, Pour Over, etc.). Otherwise, please enter Whole Bean.
    3. If this field is left blank, your Single Serve Kups will be flavors of our choosing, and your bagged coffee will be ground for Drip Grind.
  7. FREE SAMPLE TYPE: Enter either “Ground” for a 6-oz bagged ground sample, or “Single Serve Kups” for a 6-count SSK sample (for use in Keurig® machines).
  8. SUBSCRIPTION PLAN: Choose your plan from the Subscription Plan pop-up. The amount will immediately update beneath the BUY SUBSCRIPTION NOW button.
    1. Each plan includes both the quantity and a frequency interval (weekly, monthly, etc.).
    2. All plans also include a $5 local delivery & handling fee per order.
  9. BILLING INFORMATION FIELDS: Enter and double check your billing address and payment card information. The most frequent reason for a failed charge is that the billing address and zip code do not match the payment card.
  10. When you’re sure everything is correct, click the BUY SUBSCRIPTION NOW button.
  11. If the subscription is successfully processed, you will be redirected to a Thank You page and a receipt will be emailed to you. If there is an error, it will be displayed near the top of the form or by the item where the error occurred. You will need to correct the error and click the BUY SUBSCRIPTION NOW button again.

By completing the sign-up form above, you are agreeing to purchase a subscription for 12-oz bags of the current seasonal blend of Black Coffee brand coffee, or 12-count cartons of Cafejo brand coffee &/or tea, which will automatically be delivered or shipped to you at the interval you select. Your payment card will be charged for each order at the interval you selected until canceled.

  • $16.80 for every two 12-count cartons (24 kups) of Cafejo single serve kups of coffee &/or tea
